Application Process

The hiring/screening of applicants or employment includes the following procedures…

bullet Applicants must be lawfully authorized to work in the United States.

bullet Applicants are interviewed on a one-on-one basis and screened to determine their experience and skill levels.

bullet CHC requires caregivers to provide verifiable references and experience providing hands-on care.  All references will be verified prior to any applicant becoming a candidate for hire.

bullet Applicants must complete and sign an Authorization to Release form which will allow verification of the following…certifications and physical records, driving records, employment references disclosing any prior criminal convictions or licensing board’s action against them. 

bullet In addition, a criminal background check will be conducted on an applicant, prior to becoming a candidate for hire.
 

The Hiring Process

The hiring process of new CHC caregiver employees.

bullet All new hires will be required to produce the initial Fingerprint Clearance Card.  These cards are processed through the Arizona Department of Safety. Call Arizona Department of Public Safety (602) 223-2279 or fax your request to (602) 223-2947. Office hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.  http://www.azdps.gov/ (the fee is $69)

bullet All new CHC hires are required to have current CPR, First Aide, and TB tests.

bullet All new hires must furnish a Motor Vehicle Department report of their past driving record to ensure they have a clean driving record.  Along with current car insurance coverage.

bullet New hire orientation includes an extensive orientation process. This covers issues and training concerning HIPPA requirements, Code of Ethics, safety and emergency issues, Client Rights and Responsibility, Elder Abuse, zero tolerance on harassment in the work place, standards of quality service, dress code requirements, documentation standards, and a list of other important issues that pertain to the methodology in which the company expects its employees to perform at utmost professionalism and integrity.

bullet New hire that are assigned for the first time to the client will be accompanied by a manager and formally introduced.  In addition, the new hire will receive any specific training that is necessary to make the transition efficient.
